Most of Malta can be decided on the morning. A few things cannot. The Hypogeum sells out weeks ahead. Comino in August is very crowded without a booked boat, and an airport transfer booked in advance costs less than one arranged at the kerb.

What is worth booking
The Hypogeum and Comino
The Hypogeum caps visitor numbers by the hour and sells out weeks in advance. The Blue Lagoon needs a free pre-booked slot before you set foot there, whatever boat you arrive on.
Those two are the only exceptions. Almost nothing else on the islands requires it.
Attraction tickets
Transfers and anything on the water
A fixed-price airport transfer beats an unbooked taxi at midnight. A private charter beats a day boat because you choose the departure time.
In both cases you are paying for predictability rather than for access.

Beaches, cities and buses
Every bay on the islands is public and free. Valletta and Mdina are small and entirely walkable alone. Buses and ferries are turn-up-and-go.
A coach tour billed as the whole island in a day is mostly a coach.
